Are you experiencing any of the following symptoms? (Mild to moderate shortness of breath, inability to lie down because of difficulty breathing, chronic health conditions that you are having difficulty managing because of difficulty breathing, chest pains, losing consciousness) * Required

2. Are you experiencing cold or flu like symptoms, even mild ones? (Symptoms include fever, chills, cough, shortness of breath, sore throat and painful swallowing, stuffy or runny nose, loss of sense of smell, headache, muscle aches, fatigue or loss of appetite). * Required

If you answered “yes” and are experiencing ANY of the symptoms in Question #1:

Immediately inform your supervisor
Do NOT report to work and separate yourself from other people
These symptoms require immediate attention. You should call 9-1-1 immediately or go directly to your nearest emergency department. Immediately inform your supervisor

If you answered “yes” to Question #2:

Do NOT report to work
Please get assessed for COVID-19 test, and self isolate for at least 14 days or longer depending on when your symptoms started.
